How do I get a ‘form’ for the $15 rebate?

Kathy – I wasn’t replying to you (as it was 3:55am), but there is a function in the comments that alerts you when it appears that readers are leaving duplicate comments.
To get the rebate: click on the link in the post (it’s near the top under where it says “the rebate” – it will be the pink text). That will open up a new window with the rebate form. The rebate form is in .pdf form, so you will need Acrobat reader (which most people have on their computer anyway)
